1. Getting Set up for the Program:

  • Training & Registration are required for participation in this program before picking up your first order. Please click here to get signed up Your partner agreement is included in the training module. 
  • Each company should assign a Screening Supervisor Designate, who will be the
    contact person. This is the person that must complete the training and pick up the kits. We will not be able to provide kits to anybody who has not completed this training. The designate only needs to complete the training once, it is not required to be repeated to place subsequent orders.
  • The training includes a 12 minute video; please allow yourself sufficient time to watch the full video when you click through. 
  • If you have multiple businesses, please fill out one form per business.

2. Ordering Kits:

  • Please click here to place your order for screening kits; you will be able to choose a time slot from the list available; and you will get a confirmation of your order details. 
  • This site will require one-time registration so that you have access to order and report through this platform.

3. Picking Up Your Kits:

  • All visitors must complete and PASS a COVID-19 assessment before picking up kits from the premises. Please click here to self screen before you head to our office to pick up your order (no more than 12 hours in advance). If you forget to do this in advance, you will see a QR Code displayed at the entrance. 
  • We are offering curbside pick-up for the kits, so you can pull up to the front entrance of the Barrie Chamber and call to let us know that you have arrived. If you prefer to park and walk up to the door, you are welcome to do this as well, we will be following our regular distancing protocols.
  • Masks and physical distancing are mandatory during pick up.

4. Reporting Your Results

  • In order to participate in the Workplace Self Screening program, all participants must agree to immediately report the results as soon as employees have been tested.
  • Your Screening Supervisor Designate will be responsible for reporting all results.
  • Results can only be submitted through the online portal.
  • Aggregate results are shared with Health Canada and the Ministry of Health Ontario. You will not be asked for your employee names when you submit your results, and we will not be submitting your company name within our reporting structure either. Reporting is a government requirement for participation in the program; the Barrie Chamber is collecting the data purely to pass through for the program, we will not be using it for any other activities or initiatives.

Reminder

  • Anyone who receives a positive test result through the Workplace Self Screening Kit will need to make arrangements with their local Public Health Unit to receive a PCR test as soon as possible. They must also work with Public Health to determine when it is safe for them to return to work.
